How Our Postpartum Hypertension Program is Transforming Care from Day One

 Identifying Hidden Needs, Sooner

During enrollment, a patient screened positive for anxiety, reporting symptoms to us that she had not yet shared with her care team. Because she felt comfortable disclosing these symptoms confidentially, we were able to immediately connect her with the care she needed, while still in the hospital. This is just one example of how our proactive engagement helps uncover and address critical needs far beyond blood pressure monitoring.

Acting Fast to Save Lives

One patient’s markedly elevated pulse triggered an alert through the remote monitoring system. Our clinical team immediately called her, and the patient endorsed serious symptoms that she had not yet informed her obstetrician about. Our team directed her to the local Emergency Department, where she received the urgent evaluation and treatment she needed, potentially averting a major health crisis.

Offering a Lifeline in Times of Financial Strain

One patient had a temporary lapse in insurance right after birth. She shared her fear, “I was scared I’d have to choose between going into debt to take care of myself… or risking my life to save money.” Because our program is entirely free, she received the monitoring she needed without any financial burden, offering her evidence-based care and peace of mind.
